Observed cooked beef and rice for tacos at 69?F after it was cooked the day before and then cooled the beef and rice was found sitting on a shelf for an unknown amount of time out of refrigeration. The beef and rice was discarded.

Raw meat noted being stored above food not to be further cooked. Observed raw fish and raw meat being in close proximity and above vegetables and ready to eat foods in the walk in cooler.

Hand washing lavatory noted not present. Lower level food prep area does not have a dedicated hand wash lavatory supplied with soap and paper towels.
